If you're a new client, you will be directed to fill out some brief information about yourself when you go to the Schedule an Appointment page and follow the link there.
P rior to your first appointment, please click on the link below and print out the documents you will find there. Please review and sign them and bring them with you to your first appointment, along with your health insurance card if you are using insurance to pay for the visit.
